  • I am at least 21 years old.

    I own an iPhone 5, or Android 4.2 or newer with data plan.

    I am willing/able to take and pass drug, background and criminal checks.

    I understand this is a 1099 position (independant contractor), meaning I am responsible for paying my own taxes.

I understand that any misrepresentations, falsifications, or omissions on this application can be grounds for immediate denial of my appointment or removal from consideration or, if I have entered into a contract with this company, for immediate termination of that contract. I also acknowledge that I am not interviewing with Jill’s Next Door Dog Walking & Pet Services, LLC to gain knowledge of their pet sitting business in order to start my own pet sitting business.
